Thank you for participating in this research. The purpose of this study is to examine reactions to informational content about the vaccination issue. Your participation in this study will involve reading some information and answering a series of questionnaire items. The study can be completed online on any computer with an Internet connection at a time and location convenient to you. We expect it to take approximately 15-20 minutes.

All of the information you provide will be confidentially maintained to the fullest extent possible. Your answers will not be connected with your name and every possible measure will be taken to protect your privacy. While we expect academic publications to result from this study, your responses will in no way be identifiable as your own. You must be 18 years of age or older to take this survey.

Your participation is completely voluntary. Although you will receive no direct benefits, you may find the information or the survey itself interesting. Participation in this study should not put you at any risk, nor should it cause you any discomfort. However, you have the right to skip any questions or stop at any time.

It is very important that your responses are yours alone, without participation from any other person. We ask that you take the survey only once and refrain from discussing it with colleagues for 30 days. Also, during the survey DO NOT hit the return key, as some browsers interpret that as clicking the "submit" button. We also ask that you not use your browser's back button to return to earlier pages as this will likely corrupt your data.
